About the Council

Heartland Transition Council was created to promote successful transition outcomes for youth and young adults with disabilities by providing information on employment, education, training, living independently, community participation, and community support systems that influence success for all students.
Mission
This site is to serve as a resource for students, families, educators, and service providers on the topic of Transition. Our transition council has complied various resources such as informative documents, links to providers, contacts, and much more. We also look to host transition fairs and information sessions for families and educators. Our goal is to provide the needed support to see all students achieve to be the best version of themselves.
Vision
Through collaborative efforts of educators, service providers, educational organizations, and community partners, the Heartland Transition Council looks to improve transition outcomes for students in our community.
